- Required: Bachelors Degree of Social work; Preferred Master Degree in Social Work
- Experience
- Preferred: One (1) year social work experience in home care setting.
- Skills
- Ability to provide reliable transportation as required by nature of position.
- Licensure/Certification/Registration
License in Social work preferred
Eligible to sit for, and successfully pass the test of certification as a certified C ase Manager (C C M) or Accredited C are Manager (AC M) preferred
Required: Active Driver’s License
