Hospice Social Worker

We are seeking a compassionate and dedicated Hospice Social Worker to join our team. As a Hospice Social Worker, you will play a vital role in providing support and assistance to individuals with various needs. - Conduct assessments and develop individualized care plans for clients

- Provide counseling and support to individuals with developmental disabilities

- Work with families to address behavioral and emotional issues

- Provide case management services, including coordinating care and connecting clients with community resources

- Assist individuals with dementia in managing their symptoms and improving their quality of life

- Provide senior care services, including assessing needs and coordinating support services

- Strong knowledge of working with people with developmental disabilities

- Experience working with families

- Familiarity with special education systems and resources

- Proficiency in conducting psychotherapy sessions

- Knowledge of dementia care techniques and strategies

- Understanding of PTSD care and treatment approaches

- Ability to provide effective senior care services

- Strong case management skills, including assessment, planning, and coordination
