Hospice RN for Home Health

Hospice Home Health. This is a driving RN position, at Pay per visit rate.
Requirements:

• Associate degree or higher
• 1+ years of experience in ANY of the following:
  • Clinical Documentation
  • Clinical Informatics
  • Clinical Quality
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
• Valid Registered Nurse (RN) in Pennsylvania (PA)
• Must have ACLS or BLS via American Heart Association
• MUST be okay with the required travel/driving for this positio

Key Responsibilities
Lead and collaborate to provide management and delivery of patient care plans
Support patients in Carnegie/Crafton, PA and surrounding areas
Spend ~20% of the day driving to/from patient locations
Consistently meet critical deadlines for charting
Self-manage time and schedule
Schedule and Shift Details
On-call Friday, Saturday, and Sunday from 5:00pm to 10:00pm + 8 units on Saturday + 8 units on Sunday, 2 holidays per year
