What You Will Do as a Hospice RN

As a Stonerise Hospice RN Case Manager, you will plan and direct services to meet the needs of the patients and families. You will develop a working patient care plan and provide direction/supervision to paraprofessional nursing staff. Some of your primary responsibilities include:
- Completing initial assessments of patients and families to determine home care needs. Providing a complete physical assessment and history of current/previous illness.
- Developing a care plan, which establishes goals based on diagnosis, all while incorporating pain and symptom-controlled interventions.
- Supervising and directing the LPNs and/or Home Health Aides in implementing the patient care plan.
- Regularly re-evaluating patient nursing needs and revising patient care plan
- Providing health care instructions to the patient and family/caregivers per assessment and plan of care.
- Communicating with the Physicians and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers.
- Providing intimate guidance and support to patients, families, and caregivers wherever they call home.
- Acting as Case Manager when assigned by the Clinical Manager and assuming responsibility for coordinating patient care for assigned caseload.

Education, Licensure & Certification Requirements
- Active RN license in the state providing patient care
- Graduate of an accredited school of nursing
- 1 to 2 years of hospice care experience preferred
- Active CPR certification
