**Registered Nurse (RN) Case Manager in Genesee County**
Partnering with Elara Caring Hospice, as a Hospice RN Case Manager you will provide patient care across the entire continuum of care. The RN Case Manager for our Hospice service line make a meaningful impact, caring for patients wherever it is they call home. The role of Hospice RN Case Manager entails overseeing and coordinating a multidisciplinary team to deliver optimal care and outcomes for patients under hospice care. It is a bedside nursing position that affords a unique opportunity for direct patient interaction and fostering meaningful connections. Working collaboratively with your team, you will strive to enhance patient comfort, alleviate symptoms, and uphold the overarching objective of preserving dignity and providing comfort during the end-of-life phase.
Schedule: Monday through Friday from 8:00 am to 5:00 pm

What is Required?
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State Licensee as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in hospice or home health environment is preferred
- Nursing skills as defined by accepted nursing standards
- Ability to handle multiple priorities, documentation requirements, and deadlines
- Reliable transportation to perform job tasks
