Salary: Start at $88k with a $2500 Sign on Bonus!  
Service Area: Multiple Cities in a Regional Assignment
Schedule: Monday – Friday 8:00am-5:00pm with a weekend rotation

The Hospice RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The Hospice RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Qualifications:

  • 2 years’ RN experience in health care (preferably home care or hospice)
  • Experience in customer service/direct member contact field preferred
  • Experience supervising caregivers
  • Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.
  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
  • Must pass a MVR check based on Lifespark company policy
