What You’ll Do:
- Perform pre-admission evaluations for the appropriateness of services.
- Initiates the admission process by educating and informing patients/families of its services.
- Obtains informed consent
- Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
- Maintains an accurate and up-to-date record of all functions of the intake department. This includes completing the point of entry form, managing phone calls, managing the list, attending staff meetings, and verifying benefits, checking in patients
Qualifications:
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Must possess current CPR Certification
- 1 year of Hospice experience required
- Must have the ability to make independent decisions when circumstances warrant
- Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
- Must be knowledgeable of quality assessment and assurance procedures
Job Type: Full-time
Salary: $80,000.00 - $90,000.00 per year
