Hospice PTA (Physical Therapist Assistant)

We are currently seeking a compassionate and dedicated Home Health Physical Therapist Assistant (PTA) to join our team. As a Home Health PTA, you will play a vital role in providing essential support to the Physical Therapist in delivering personalized and effective rehabilitation care to patients within their homes. Your assistance will significantly contribute to enhancing patients' mobility, function, and overall well-being.

Responsibilities:

1. Assistance in Patient Care:

   • Aid in conducting patient assessments and documenting information to assist in creating individualized treatment plans.
   • Support the implementation of therapeutic exercises and techniques under the guidance of the supervising Physical Therapist.

2. Support in Therapeutic Interventions:

   • Assist patients with prescribed exercises, therapeutic activities, and functional training to improve mobility and strength.
   • Provide feedback on patient response to treatments and report any concerns to the supervising Physical Therapist.

3. Documentation and Record-Keeping:

   • Maintain accurate and detailed records of patient progress, treatments administered, and changes in their conditions.
   • Ensure compliance with documentation standards and regulatory requirements.

4. Collaboration and Support:

   • Collaborate closely with the physical therapy team, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care.
   • Participate in team discussions and assist in adjusting treatment plans based on patient progress.

5. Patient Education:

   • Instruct patients and their families on prescribed exercises, safe mobility techniques, and the use of assistive devices to facilitate ongoing progress.
   • Offer support and encouragement to patients throughout their rehabilitation journey.

Role Requirements:

1. Education and Certification: Completion of a Physical Therapist Assistant program from an accredited institution. Valid state PTA license.

2. Experience: Previous experience in a home health setting or relevant healthcare environment is preferred.

3. Compassion: Demonstrated empathy, sensitivity, and a patient-centered approach to care.

4. Communication Skills: Strong verbal and written communication skills to effectively interact with patients, families, and the healthcare team.

5. Adaptability: Ability to work independently and adapt to various home environments while prioritizing patient safety.

6. Problem-Solving Skills: Critical thinking abilities to assist in adjusting treatment plans based on patient progress and challenges.

7. Ethical Conduct: Adherence to high ethical standards, maintaining patient confidentiality, and respecting their autonomy.
