Job Description
Social work services are provided by a qualified Social Worker under the direction of a physician and in accordance with a plan of care. Medical Social Worker offers counseling services to provide information and support during all facets of the hospice process to help the patient/family cope with advanced/chronic illness.

Primary Job Responsibilities
- Performs/documents psychosocial assessment for patients.
- Provides emotional support for patients and families facing advanced or chronic illness.
- Educate patients and families on role of hospice care.
- Assist with placement and discharge planning needs.
- Assess bereavement needs as necessary.
- Provides short-term counseling and crisis intervention.
- Links patients and families to appropriate community resources.
- Collaborates with the interdisciplinary hospice team.
Requirements
- Master of Social Work required
- LSW preferred
- Minimum of 1 year hospice, palliative and/or home care experience preferred
- Minimum of 1 year healthcare experience required
- Driver's License and Car Insurance in good standing
- Reliable vehicle
- Possesses the ability to follow oral and written instructions
- Possesses the ability to communicate effectively with all members of the care team
- Possesses good organizational skills
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
