SalaryJob Summary: Provides skilled occupational therapy to patients in the home setting. Assess, plan, implement and evaluate treatment plans and interventions. Monitors the patient response to treatment and progress towards goals in compliance with physicians’ orders and the plan of care.
Qualifications/ Educational Requirements:
1. Successful completion of an Occupational Therapy program at a school of Occupational therapy approved by the American Occupational Therapy Association.
2. Licensed as an Occupational Therapist in the state of practice.
3. Minimum one year of experience as an Occupational Therapist preferred.
4. Maintains current CPR certification.
Responsibilities/essential functions: Must be able to perform the following essential job functions with or without reasonable accommodations.
1. Obtains patient medical history on evaluation.
2. Assess, plan, implement and evaluate occupational therapy needs of the patient.
3. Provides skilled occupational therapy in accordance with physician's orders.
4. Implements therapy in accordance with the patient plan of care
5. Coordinates care and communicates with the hospice interdisciplinary team.
6. Revises interventions and treatments as necessary.
7. Evaluates patients’ response to therapy and communicates changes to the interdisciplinary team.
8. Documents care in the medical record. Documents at point of care.
9. Educates the patient, family and caregivers in the use of assistive devices and durable medical equipment.
11. Completes education and participates in educational in-services.
12. Participates in the Quality Assurance Performance Improvement program.
