Hospice Nurse Practitioner
Location: Eugene, OR
Shift: Full-Time, M-F, with call coverage
Salary: $109,280 - $163,900 per year
Burns Recruitment is partnered with a leading healthcare organization in Eugene, Oregon, to find a dedicated Hospice Nurse Practitioner.
Position Overview:
As a Hospice Nurse Practitioner, you will be part of a multi-disciplinary clinical team, providing clinical assessment for patients with advanced and progressive medical conditions referred to hospice and palliative care. You will assist in evaluating and treating patients in the community or at the Pete Moore Hospice House. This role involves patient interviews, diagnostic data collection and interpretation, physical examinations, and specialized procedures under the delegation of the Medical Director. You will also make house calls for chronically ill adults at their place of residence and the Pete Moore Hospice House. Collaborating closely with the Medical Director, you will share call coverage during weekends and evenings.
Education & License Requirements:
- Graduate of an accredited school of professional nursing with advanced training as a family or emergency Nurse Practitioner.
- Current State of Oregon Nurse Practitioner licensure.
- CPR certification.
- Prescriptive drug privileges.
- DEA number.
Minimum Qualifications & Experience:
- Two years of clinical experience with a hospice and palliative care background preferred.
- Strong verbal and written communication, problem-solving, and decision-making skills, with expertise in planning and organizing.
- Ability to assess and effectively resolve difficult situations.
- Maintain a high standard of professionalism in all clinical activities, working with patients, family, and caregivers.
- Ability to work independently and within a team framework.
Essential Job Functions:
- Provide bedside palliative care to patients.
- Apply various clinical skills, including health status assessment, disease screening, treatment provision, patient education, and healthcare guidance.
- Diagnose, treat, and manage acute and chronic health conditions.
- Order, perform, and interpret laboratory and radiology tests within the scope of professional practice.
- Ensure complete and timely patient care documentation according to regulations.
- Conduct comprehensive adult and geriatric exams, adjust medications, establish treatment plans, educate patients and nursing staff, coordinate care, participate in coordinated care rounds, and assist with discharge planning for optimal patient outcomes.
- Prescribe medications, including controlled substances, as delegated and licensed.
- Perform therapeutic or corrective measures as indicated, including urgent care assessment and treatment.
- Consult with the interdisciplinary team, including the Medical Director, as needed.
- Inform primary physicians, specialists, and the care team of services provided, collaborating with others as necessary.
- Provide quality clinical services with a patient-oriented bedside manner.
- Foster a positive, supportive, and energetic team atmosphere.
- Demonstrate computer proficiency with knowledge of EMR documentation and utilities.
- Function within the Scope of Practice for Nurse Practitioners outlined by the Oregon State Board of Nursing.
- Assist with additional duties and responsibilities as assigned.
