Medical Social Worker - Bronx county
Responsibilities:
We are looking for an experienced Social Worker, able to provide psychosocial assessments of and supportive interventions for hospice patients and their families. As a LMSW, you will be an integral member of the hospice interdisciplinary team. Your empathy and compassion will be needed wherever our patients live: SNFs, Inpatient Units, and their homes. Your team's service area is the Bronx County, NY.
Qualifications:
- LMSW/LCSW in NY State
- Covid-19 Vaccinated
- 2+ years of SW experience (healthcare) preferred
- Demonstrated experience in discharge planning
- Ability to work with limited supervision
- Strong interpersonal skills
- Collaborative and team-oriented personality
- Ability to work with a diverse patient population
- Bilingual English/Spanish speaking preferred
- Reliable motor vehicle required
- Computer proficiency and previous experience with EMR systems a plus
