Licensed Vocational Nurse (In-Office Support)

Job responsibilities include:
- Collaborates with RN case managers and clinical leadership to coordinate nurse and hospice aide visits and ensures patient visits are completed according to the plan of care.
- Accurately maintains the electronic schedule for all clinical team members as assigned.
- Responsible for ensuring all unplanned visits are covered by appropriate team members.
- Assist with patient visits in periods of staffing shortages and critical patient need.
- Answers and assists with incoming calls and triages them with RN supervision.
- Reviews patient records, to include most recent clinical notes, medication lists and care plan prior to giving directions.
- Completes Electronic Medical Records (EMR) in a timely and efficient manner in accordance to department policies and procedures.
- Cross trains as Admissions Coordinator to assist during times of high need, or position coverage.
- Initiates communication with Case Managers, IDT members, doctors and those necessary to communicate with in order to meet the needs of the caller.
- Assists leadership with quality assurance performance improvement initiatives.
- Assist with administering flu injections or TB tests for team members.
- Additional duties as requested.
Minimum qualification requirements:
- Current license to practice as a Licensed vocational nurse in the State of California
- Valid Driver's License and proof of Auto Insurance Coverage
- Reliable transportation to the office and field visits as needed
- Ability to respond to needs of patients/families/caregivers in varied settings and to function effectively in a nursing facility environment
- Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
- Hospice experience highly preferred.
