Requirements

Job Functions:

  • Provides skilled and palliative nursing in the home as ordered on the plan of treatment and nursing care plan.
  • Evaluates the significance of clinical data findings and communicates any pertinent information about the patient's clinical status and ongoing needs to others in a timely fashion.
  • Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Ability to adequately evaluate and re-evaluate pain.
  • Utilizes appropriate pain management techniques.
  • Educates the patient and family regarding pain management. Notifies the RNCM when pain is not manageable on the current medications.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient's clinical position.

Experience:

  • One year Hospice preferred, but not necessary

Credentials:

  • Licensed Practical Nurse in State of Indiana
  • Proof of current classroom CPR certification
  • Hepatitis profile and TB vaccination record
  • Current driver's license
  • Must have reliable transportation and insurance
