Iowa City Hospice is seeking a dedicated and compassionateHospital Liaison Nurse to join our healthcare team. Because this position is responsible for the referral and admission process of patients and families in the Iowa City hospital setting, special considerations will be made for nurses currently working in those settings. Our Liaison nurses enjoy using interpersonal communication skills and their passionate understanding for the hospice philosophy to network and educate others about palliative measures that support both acute and crisis symptom management while caring for patients and families with life-limiting needs within the hospital setting. They coordinate and facilitate communication between patients, families, and healthcare providers to ensure the highest quality of care is provided. This role requires strong working knowledge of our local hospital settings, excellent communication skills, strong clinical knowledge, and the ability to work collaboratively with hospital staff and our multidisciplinary team.

Essential Job Functions (including but not limited to):

  • Collaborate with healthcare providers to ensure timely and appropriate referrals, transfers, and discharges
  • Develops early and appropriate referrals by creating and sustaining relationships in the medical community.
  • Maintains consistent and appropriate contact with active referrals within the hospital setting.
  • Participates in the preparation and accomplishment of marketing and outreach goals.
  • Coordinates the referral and admission process in the hospital setting.
  • Determines patient’s appropriateness for services consistent with Iowa City Hospice policies and admission criteria.
  • Demonstrates exceptional written and verbal communication standards to advance cooperative working relationships with Iowa City Hospice staff, management and care partners.
  • Assumes initial responsibility for assessing patient/family needs and consulting with attending physician, hospital staff and hospice staff members.
  • Obtains information on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate into the plan of care.
  • Assists with continuity of care and discharge planning in hospitals and facilitate and ensure smooth transition of new patients to the primary care team.
  • Provides care to the patients in the hospital, either at the respite or general inpatient level of care.
  • Attends educational opportunities and provide Iowa City Hospice educational programs to hospitals and clinics.
  • Shares in providing 24-hour, seven-day-a week coverage to patients/families.
  • Ensures effective use of resources.
  • Serves as an educational source to staff, Care Partners and the Community.
  • Ensures implementation of strategic goals
  • Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
  • Participates in Iowa City Hospice’s success by ensuring access to our exceptional care.
  • Supports and promote Iowa City Hospice’s mission to lead in providing exceptional patient-centered end-of-life care and grief support to anyone affected by a terminal illness.
  • Participates in the organization’s quality assessment and performance improvement program.
  • Performs other duties as assigned.

Requirements:

Requirements:

- Current and valid Iowa state nursing license

- Bachelor's degree in Nursing (BSN) preferred

- Minimum of 3 years of clinical nursing experience preferred

- Special consideration to nurses with direct work experience at Mercy IC or UIHC

- Excellent communication and interpersonal skills; ability to communicate effectively in the English language both verbally and in

writing

- US work authorization (Required)

- Strong clinical knowledge and critical thinking abilities

- Ability to work effectively in a fast-paced, high-stress environment

- Demonstrated ability to work collaboratively with a multidisciplinary team

- Proficiency in electronic medical records (EMR) systemsRN degree required; BSN degree preferred.

- Proof of IA Driver License and current Auto insurance coverage.

- Passionate about hospice and home care with a genuine desire to help others

- All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List

- Reliable, professional and able to start soon after background check is completed

- 2 step TB testing is required.

- First Aid training

- Mandatory Reporter Training

- Iowa City Hospice requires applicable Advanced Certified Hospice and Palliative Nurse (ACHPN®) by the 3rd year of employment.
