Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.
The schedule for this position is 1030-1900; Manager On-Call; occasional weekends

Responsibilities:
  • Coordinate interviewing/hiring and orientation of new clinical staff.
  • Manage various personnel actions including, but not limited to hiring, orientation, performance appraisals, promotions and scheduling; counseling / mentoring, and implementation of formal discipline of clinical staff, as needed. Ensures Timecard completion and approval for assigned staff.
  • Manages staff schedules in collaboration with the Senior Administrative Assistant to ensure adequate staffing coverage for all shifts. This includes enforcement of PTO and Holiday policies, Low census protocol, and appropriate staffing ratios based on the needs of unit.
  • In collaboration with the Hospice Home IPU Clinical Manager and AVP of Hospice, Develop and implement processes to enhance clinical operations and practices for ongoing quality improvement.
  • Oversee medication room operations and regulatory compliance related to narcotics and other medications.
  • In collaboration with the Clinical Manager of the Hospice Home, sets expectations for daily admissions / census goals based on staffing and collaborates with the Charge Nurse and the Admissions Coordinator to ensure patients get timely access to care at the IPU.
  • Oversee clinical compliance with all regulatory and accreditation needs of the agency. This includes collaboration with the Quality Team to follow up on audit findings. Participates in PIPs and or Quality Improvement Projects as needed.
  • Ensure staff documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures, and regulatory requirements. Co-hosts IDG weekly and ensures that all necessary disciplines are represented, and that all documentation is complete for each patient record.

Requirements:
  • RN with current N.C. license.
  • Two years of previous supervisory experience preferred.
  • 5 years recent clinical experience with hospice, home health, or equivalent.
  • Current CPR certification.
  • Thorough understanding of Hospice clinical interventions and protocols.
  • Knowledgeable about reimbursement sources, Medicare/Medicaid regulations, licensing laws and accreditation standards.
  • Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
