Responsibilities:- Maintain psychological and emotional support for patients and family members - provide quality companionship
- Ambulate patients as indicated in the care plan, and help patients complete exercises and physical therapy as needed
- Travel with patients to health care service providers, doctors’ offices, grocery stores, or for other errands, as needed
- Supervise medications and assist patient in organizing, refilling, and taking medications, as well as meal planning and preparation
- Reinforce accurate and up-to-date care records and work with the case manager to implement the patient care plan
- 1-3 years of experience as an HHA/ home health aide, personal caregiver, nursing assistant, or personal care aid for a home health agency, at patient homes or care facilities settings is needed
- Current CPR certification and first aid training is needed and you must be able to pass a full employment background check, drug test, and physical
- Compassionate, kind, and empathetic personality is essential, with superb organization and communications skills
- Physical requirements include being able to lift up to 100 lbs, may be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed
- Conclusion of home health aide training programs from community colleges are acceptable - Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is a bonus
