Hospice CNA

```Job Overview```
We are seeking a compassionate and dedicated Hospice CNA to join our team. As a Hospice CNA, you will provide essential care and support to patients in their end-of-life journey. This is a rewarding opportunity to make a meaningful impact on the lives of individuals and their families during a challenging time.

```Duties```
- Assist patients with activities of daily living, including bathing, grooming, and dressing
- Administer medications as directed by healthcare professionals
- Monitor vital signs and report any changes or concerns to the nursing staff
- Provide emotional support and companionship to patients and their families
- Assist with light housekeeping tasks to maintain a clean and comfortable environment
- Document patient care activities accurately and timely in medical records
- Collaborate with the interdisciplinary team to develop and implement individualized care plans

```Qualifications```
- Valid CNA certification
- Experience in critical care, dementia care, behavioral health, or senior care is preferred
- Basic life support (BLS) certification
- Knowledge of medical terminology and ability to interpret medical records
- Proficiency in eClinicalWorks or similar electronic health record system is a plus
- Ability to lift and transfer patients safely
- Excellent communication and interpersonal skills

Pay: $30.00 - $40.00 per hour

Expected hours: 1 – 25 per week

• Long term care

Standard shift:

• Day shift

Weekly schedule:

• Monday to Friday
• Weekends as needed

Ability to Relocate:

• Phoenix, AZ: Relocate before starting work (Required)
