The Role
As our Hospice Clinical Supervisor, you will play a crucial role in ensuring the delivery of high-quality end-of-life-care to patients and their families. This role balances management and field work, providing you with an opportunity to utilize and enhance your clinical skills while honing leadership expertise. You will ensure our services align with state and federal regulations, contributing to a high standard of care.
Responsibilities
- Supervise and support a team of hospice nurses, hospice aides, and other clinical staff.
- Review and evaluate patient care plans and ensure they are implemented effectively.
- Monitor patient care to ensure it aligns with the individualized hospice care plan.
- Collaborate with the interdisciplinary team, including physicians, social workers, and chaplains, to provide holistic patient care.
- Ensure that care provided meets all regulatory and accreditation standards, such as Medicare and Medicaid guidelines.
- Address and resolve any clinical issues, concerns, or deviations from care standards.
- Act as a resource for patients and their families, providing guidance and emotional support.
- Assist with difficult patient cases and family communication as needed.
- Stay up-to-date on changes in hospice care regulations and ensure the organization's compliance with all legal requirements.
- Assist in the preparation for surveys and audits.
Your Qualifications
- 3-5 years of healthcare experience, preferably in Home Health or Hospice.
- 2 years of supervisory experience is preferred.
- Licensed clinician in the State of PA.
- Knowledge of healthcare regulations and our agency's policies and procedures.
- Excellent leadership skills and the ability to exercise independent judgment.
