Guardian Angel Hospice desires a Chaplain to join our experienced pastoral care staff in providing spiritual care to our patients, families, staff, and community. The Hospice Chaplain will work closely with the interdisciplinary team members in providing spiritual care in homes, long-term care facilities, and inpatient settings.
The Chaplain will:
- Demonstrate competency in dealing with grief, death, and the process of dying.
- Conduct assessments of patients/families' spiritual needs.
- Provide direct spiritual services to patients and their families according to their belief system and practice.
- Ensure the appropriate spiritual services are provided in a timely manner by either providing such to the patient/family or coordinating the provision of services with community clergy or spiritual counselors.
- Interact with the hospice team members to identify and address patient care needs and develop the Plan of Care.
- Provide documentation in the Electronic Medical Record utilizing good written communication skills.
- Conduct religious services for patients/families as well as hospice and facility staff as requested.
- Serves as a liaison and support to community clergy and spiritual counselors.
Job Requirements
- Graduate of an accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree).
- Certification by the American Associate of Pastoral Counselors, Association of Professional Chaplains, Nation Association of Catholic Chaplains, or similar professional organization, or be eligible for certification.
- Experience in end-of-life care preferred.
- Ability to work as a member of an interdisciplinary team.
- Comfort in an interfaith setting.
- Ability to manage your schedule effectively and efficiently.
- Must have reliable transportation, a valid driver's license, and be willing to travel within the company's service area.
