Job Summary
The Hospice Registered Nurse (RN) Case Manager is responsible for providing end-of-life care to patients and their families. The RN Case Manager will work closely with the interdisciplinary team to develop and implement a plan of care that meets the physical, emotional, and spiritual needs of the patient and family. The RN Case Manager will also provide education and support to the patient and family throughout the hospice journey.
Responsibilities
- Assess and evaluate the patient's physical, emotional, and spiritual needs
- Develop and implement a plan of care that meets the patient's needs
- Coordinate care with the interdisciplinary team, including physicians, social workers, chaplains, and volunteers
- Provide education and support to the patient and family
- Manage symptoms and provide comfort measures
- Document all care provided in the electronic medical record
- Participate in on-call rotation as needed
Qualifications
- Current RN license in the state of Pennsylvania.
- Preferably one (1) year of nursing experience as a Home Care Nurse.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong critical thinking and problem-solving skills.
- Ability to manage multiple priorities and meet deadlines.
- Has a reliable means of transportation and proof of insurance.
