Hospice of the Piedmont is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our East Team which serves counties East of Charlottesville, VA. You will be providing exceptional hospice care to patients in their homes during their final stages of life. As a CNA in our hospice care program, you will play a crucial role in providing comfort, support, and assistance to patients and their families during this challenging time. Responsibilities:
- Provide direct patient care in accordance with the care plan developed by the interdisciplinary team, including assistance with activities of daily living (ADLs), personal hygiene, and comfort measures.
- Observe and report any changes in patient condition or behavior to the hospice nurse.
- Assist with mobility and transfer of patients as needed.
- Document all care provided and any changes in patient condition accurately and timely.
- Provide emotional support and companionship to patients and their families.
- Collaborate with the interdisciplinary team to ensure the highest quality of care.
- Maintain a clean and safe environment for patients.
- Respect and uphold patient rights and confidentiality.

Qualifications:
- Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
- Minimum of 3 years’ experience as a registered nurse within the last 5 years.
- Highly knowledgeable in pain management, symptom control, crisis intervention and family support.
- Knowledge of hospice regulations, practices, and standards of care with a demonstrated ability to manage all aspects of terminal care.
- Excellent clinical assessment and critical thinking skills, emotionally mature with the ability to cope with family stressors and tolerance of individual lifestyles.
- Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
- Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
- Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
- Highly organized, self-motivated with the ability to articulate information with a diverse population, and proficient in using electronic medical records (EMR) and other healthcare software.
