This is a community-based role and will require regular travel through the borough of choice.
The compensation structure comprises both base pay and bonus pay.

The Admission RN coordinates the registration and pre-admission screening of all referrals, including infusion cases; assesses needs and communicates preadmission data to Hospice of New York (“HONY”) programs and clients.
Assesses/evaluates/identifies the physical, social, and psychological needs of referred clients.
Provides/performs clinical assessment based on clinical judgment.
Works under general supervision.
QUALIFICATIONS:
1. Registered nurse currently licensed in the New York state required.
2. Baccalaureate Degree in nursing from an approved program; or
a. Baccalaureate Degree in nursing from a non-approved program supplemented by content which can be equated to the approved program; or
b. Registered professional nurse with two years’ experience in either home health care or Hospice nursing.
3. Minimum one year Med/Surgical or other professional nursing experience relevant to assigned case management preferred.
4. IV Therapy Certification accredited by a nationally recognized accreditation body may be preferred based on operational needs. If supporting infusion program:
a. Minimum of one year of experience in infusion therapy nursing required if supporting Infusion program.
b. Proficiency in venous access and related infusion therapy skills required.
5. Clinical competence in nursing and the ability to multitask in a fast-paced environment, listen attentively/evaluate calls/use judgment in a time sensitive environment required.
6. Effective oral/written communication, customer service and interpersonal skills required.
7. Knowledge of Computer operation, computer generated clinical records experience preferred.
8. Demonstrated commitment to Hospice philosophy of care.
9. Must have a valid driver’s license, motor vehicle, and minimum state required liability automobile insurance.
10. Bilingual skills may be required, as determined by operational needs.

Pay: $90,000.00 - $140,000.00 per year
