We are in search of a Nurse Educator that will build relationships with nursing home residents to review their medical status, provide health education about their medical conditions, and assist with skill building for self-management tasks. After clients move into the community, the H.O.P.E. (Home Options Path to Empowerment) Nurse Educator visits clients in their homes to assist with medication set up, to support individual health goals, to continue to provide health education and assist with skill building, and to provide an evaluation/assessment after unexpected ER visits and hospitalizations.

DUTIES AND RESPONSIBILITIES:
- Educate nursing home residents about their medical conditions, medications/treatments, and ongoing health care needs to support independent living
- Provide training and education about medical conditions and issues such as diabetes, wound care, medication adherence, etc.
- Engage members in discussion of adherence to personal health plans
- Visit nursing home residents while they reside in the nursing home and after they move into the community
- Makes home visits after unexpected ER visits and/or hospitalizations
- Support development of assessments and comprehensive service plans and provide guidance to HOPE team on health issues
- Makes referrals to community agencies as appropriate.
- Collaborates with partner agencies as needed
- Follows all program standards and procedures.
- Completes all required documentation according to program standards.
- Refers to Adult Protective Services (APS) program or Long-Term Care Ombudsman when abuse, neglect or exploitation is suspected.
QUALIFICATIONS:
· Associate degree in nursing required, BSN preferred
· RN license
· Minimum of 1 year experience in providing services to older adults
· Experience in working with clients with severe mental health conditions, drug/alcohol misuse and/or complicated physical illness preferred.
· Excellent written and oral communication skills with demonstrated ability to engage and motivate clients
· Computer skills required: Microsoft Office Suite; ability to learn proprietary programs
· Other skills required:
o Knowledge of community supportive services
o Sensitivity to cultural diversity
o Ability to work with people who have multi-faceted needs
o Current driver’s license and access to a vehicle
o Experience with Motivational Interviewing, preferred
o Experience with Colbert and/or Williams consent decrees, preferred
o Experience with hospice, home health care and/or community care management, preferred
