Easterseals NH services include early childhood centers of excellence, medical rehabilitation, camping and recreation, vocational services, senior services, substance use treatment, adult day programs, community-based services, individual service options, transportation services, residential service options, and veterans services.
We are looking for a Homemakers & Health Services Nurse - Registered Nurse to join our team! The individual in this key position is responsible for conducting assessments, supervision, and training in accordance with licensing requirements and agency policies. Additional responsibilites include but are not limited to:
- Provide nursing oversight for Home Health Aide and Personal Care Services throughout NH and conduct initial and ongoing health assessments for individuals served.
- Develop, approve and monitor care plans in collaboration with Care Managers and assist in coordinating referrals to community agencies.
- Provide clinical training for Personal Care Service Providers, and clinical supervision and training for Licensed Nursing Assistants.
- Promote safety awareness for staff and individuals served.
- Record data utilizing electronic documentation systems per state requirements & department policies.
Qualifications:
- Registered Nurse license with the New Hampshire Board of Nursing required
- Five (5) years of experience working in home care preferred
- Three (3) years of supervisory experience preferred
- Strong computer, organization, communication and problem-solving skills
- Willingness to travel and work flexible hours to meet the needs of the program
- Valid driver's license and reliable transportation
Compensation: $35.00 hourly
Hours: Per diem
