Position Overview:
Under the supervision of the ACCESS Senior Manager, the Homeless Cohort Manager will oversee a team of coordinated care professionals to achieve the goals of the Cohort of ACCESS Harris County. The Cohort Manager will lead and supervise the coordinated care team, train new staff members, provide logistical guidance and support, maintain a working knowledge of and compliance with ACCESS Harris objectives and procedures, and coordinate with additional supervisors to deliver quality participant care. The Cohort Manager will also oversee outreach efforts to increase access, communication, community engagement, and education for residents of Harris County.
Duties and Responsibilities:
  • Provide direction to employees within the team by organizing, maintaining staffing levels, ensuring new employee training, performance management, participate in regular meetings with Harris County Public Health staff to review and assess program progress, assess the relevance and adequacy of the plan as it is developed to determine other priority needs and goals etc.
  • Oversee operations/activities of participants care-coordination team; responsible for providing ongoing case management services for participants enrolled in ACCESS; works with participants to enhance stability and overall well-being by addressing specific needs that may have contributed to the identified vulnerability; assists with internal and external referrals for medical, substance use, mental health, parenting, employment, housing needs, and other needs related to social determinants of health
  • Be a main point of contact for ACCESS Senior Manager, care coordination team, and safety net department appointed contacts; facilitates case coordination and information sharing amongst safety net departments, community-based organizations, and any additional stakeholders; provide continuous feedback to the Care Coordination Team during case conference; maintains case records and documentation as appropriate
  • Responsible for workflow facilitation thru the effective use of the care coordination team, increasing productivity and efficiency to meet ACCESS Harris program initiatives; provide trauma-responsive case management services, effectively manage crisis situations through assessing, deciding, and acting in accordance with standards and laws in emergencies involving the health and safety of participants being compromised; provide empathetic advice and guidance to participants to ensure participant satisfaction and program completion
  • Evaluate, document, and continuously improve team performance and program development
  • Conduct site/home visits as needed to complete initial assessments on new participants and provide follow-up to existing participants; ensure the safety and basic needs of the participant are met in an appropriate and caring manner
  • Monitors status reports, metrics, or ad hoc reports, as requested by leadership; facilitates case coordination and information sharing amongst safety net departments, community-based organizations, and any additional stakeholders
  • Create consistent outreach plans to provide education and awareness of ACCESS Harris; assist with coordinating and leading cohort teams at local events, community gatherings, workshops, and training and to community leaders, local politicians, community-based organizations
  • Develop and maintain knowledge of operations and functions of Harris County government associated with achieving the goals and vision of Harris County Public Health through ACCESS Harris and related services; Conduct a community assessment to identify resources and community organizations/groups that can assist in furthering the mission of ACCESS and facilitate implementation of the strategies identified in the plan, emphasizing the maximum engagement of community residents’ existing community services
  • Ensure compliance with internal policies, procedures, protocols, and external guidelines, laws, and regulations; develop a strategy to increase resident participation in community events and amenities
  • Performs other duties/responsibilities or special projects as necessary

Requirements

Education:
  • Bachelor's degree in Business Administration, Healthcare Administration, Public Health, or a related field
Experience:
  • Two (2) years of supervisory experience leading teams to reach challenging goals and objectives in the social service space
  • One (3) year experience working as Housing Case Manager or in Case Management
  • One (1) year experience working with coordinate care
Computer Skills:
  • Proficient in Microsoft Office Suite software, including 365 programs

Work Environment:
  • This job operates in a professional office environment with frequent occurrences of community outreach events, volunteer activities, and site visits to include the residence of participants.
  • This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands:
  • This position is mostly sedentary but may be required to sit or stand, use manual dexerity and work in an office or indoor environment.
  • May occasionally lift and move up to 25 pounds and occasionally up to 50 pounds.
