We have an opening for a HomeCare Supervisor who can answer yes to these questions. HomeCare Supervisor's Benefits include:

HomeCare Supervisor's Primary Responsibilities:

• Familiarize with SYNERGY HomeCare Processes and Procedures on deliverying HomeCare Services
• Participate and/or assist in Home Assessments working together with Community Liason / Sales Team
• Seek and understand the customer's care plan and required details from Community Liason / Sales Team
• Own the execution of the care plan for the cusotmer
• Confirm and update caregivers' availability including vacation, sick time, and holidays
• Document skills and attributes of caregivers to ensure strong matching to clients
• Follow up with new caregiver hires to explain the scheduling process
• Establish a great rapport with caregivers and clients
• Schedules shifts and hours by matching caregiver qualifications and availability to client’s needs.
• Watching scheduled visit lists to be sure that all caregiver shifts are completed for our clients.
• Proactively adjust or fill in the shifts whenever there is a change in schedule.
• Communicates new assignments and/or schedule changes to Caregivers and Customers
• Update scheduling data to initiate accurate payroll and billing processes.
• Participate in after hours, on-call duty on a rotating basis.
• Work with Client Care Manager and HR Coordinator/Recruiter to assist with issues resolutions
• Document changes in client’s conditions.
• Document caregiver call outs, refusal of shifts, and any other changes including explanations as to why changes were made.
• Documents all quality calls and visits.
• Contribute to the growth of the company.
• Reporting of accountabilities and recommendations on services improvements
• Recommend caregivers for monthly recognition

HomeCare Supervisor's Requirements:

• Experience – Two years experience in home care supervision role preferred
• Education – High school diploma / Associate Degree / Bachelors Degree / Health or Home Care Industry Certifications or Licensure
• Skills – Exceptional Customer Service Skills, Time Management Skills, Communication Skills
