The Home Visit Specialist is responsible for conducting Home Visits to patients' homes to build and strengthen relationships, implement the start of homecare, and gather necessary compliance pieces. They will ensure patients have the proper durable medical equipment in their homes and provide necessary training to patients, home health aides, and nurses as required. They will conduct weekly check-ins with all team members, nurses, and home health aides involved in caring for each patient under your management. This individual will be a valued member of a care team, ensuring client's needs are met in relation to their medical conditions while collaborating with the home health aide, and nursing staff, focusing on each client's plan of care. Additionally, this individual will be trained in a secondary specialty of Physician Visit Specialist and will be responsible for representing our company and our clients at their Giving-related physician appointments. This individual will provide information to physicians and our clients about the program that we work with, ensuring a personalized experience for each client through patient advocacy. This individual must possess strong communication skills, both written and oral, along with strong interpersonal and computer skills.

Qualifications:

  • Experience in Case Management/Social Work (will carry a caseload)
  • Proficiency in Medical/Medical Terminology
  • Active engagement in a collaborative team environment
  • Strong reading and writing skills
  • Adaptability and Flexibility with a mind towards achieving goals
  • Excellent organizational skills
  • Possession of a strong value system, showing loyalty and dedication to serving humanity
  • Understand HIPAA and maintain confidentiality
  • Holding a valid driver's license, car insurance, and dependable transportation for patient visits and doctor appointments
  • Completion and passing of a mandatory background check by the Department of Health
Responsibilities:
  • Must be willing to drive daily, drive time is included in an 8-hour shift, mileage reimbursement
  • Frequent in-home patient care visits
  • Collaborate with patients, home health aides, and nurses to handle medical documentation for their homecare services
  • Conduct visits to patients' homes to build and strengthen relationships, implement the start of homecare, and gather necessary compliance pieces
  • Provide necessary training to patients, home health aides, and nurses as required
  • Conduct weekly check-ins with all team members, nurses, and home health aides involved in caring for each patient under your management
  • Collaborate and manage a caseload of 100-150 patients as a team
