Position Overview:

We are seeking a highly skilled and compassionate Home Hemodialysis Nurseto join our dynamic team. The ideal candidate will be responsible for delivering specialized hemodialysis treatments to patients in their homes, ensuring the highest level of care, comfort, and safety.

Responsibilities:

• Conduct comprehensive assessments of patients' health status and dialysis needs.
• Develop individualized dialysis treatment plans in collaboration with the interdisciplinary team.
• Perform hemodialysis procedures in patients' homes, adhering to established protocols and safety standards.
• Monitor patients' vital signs, fluid balance, and response to treatment during each session.
• Educate patients and their families on home dialysis procedures, equipment use, and self-care techniques.
• Provide ongoing support, counseling, and encouragement to patients to promote treatment adherence and positive outcomes.
• Document all patient encounters, treatment plans, and outcomes accurately and in a timely manner.
• Collaborate with physicians, nurse practitioners, and other healthcare professionals to optimize patient care and outcomes.

Qualifications:

• Current RN licensure in the state of Texas
• Specific experience in Home Hemodialysis required.
• Proficiency in operating and troubleshooting dialysis equipment.
• Excellent assessment, critical thinking, and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Strong commitment to patient-centered care, compassion, and empathy.
• Valid driver's license and reliable transportation.

Pay: $42.00 - $52.00 per hour
