Compensation:
RNs: $60-$90 per visit
Start of Care Visits: $75-$90 based on experience
Follow-Up Visits: $60-$65 based on experience
We are actively seeking compassionate and experienced Home Health Nurses to join our dedicated team serving the North Chicagoland Area. As an RN with previous Home Healthcare experience, you will play a crucial role in providing high-quality care to our patients in the comfort of their homes.
Responsibilities:
- Conduct in-home patient assessments and create personalized care plans.
- Deliver skilled nursing care according to individual patient needs.
- Monitor and assess patient conditions, communicating changes to the healthcare team.
- Complete accurate and timely documentation.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
- Educate patients and their families on proper care procedures.
Qualifications:
- Active Registered Nurse (RN) license in the state of Illinois.
- Previous experience in Home Health nursing.
- Proficient in completing documentation in a timely manner.
- Reliable transportation with valid auto insurance.
- Ability to create and manage your own schedule.
- Flexibility to work some weekends.
