Job Details:
- Location: Columbus, GA
- Schedule: Full-time, 40 hours per week, Monday to Friday, 8:00 am to 5:00 pm
- Occasional weekends & on-call.

Responsibilities:
- Conduct comprehensive patient assessments to identify healthcare needs and develop individualized care plans.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and infusion therapy.
- Perform patient education on disease management, medication administration, and lifestyle modifications to promote optimal health outcomes.
- Coordinate care with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care.
- Utilize electronic health record systems for documentation of patient encounters, treatments, and progress notes.
- Monitor patient progress and make necessary adjustments to care plans based on evolving healthcare needs.
- Adhere to all regulatory guidelines and agency policies to maintain high-quality standards of care.
Requirements:
- Valid Registered Nurse (RN) license in the state of Georgia.
- Minimum of 2 years of Registered Nurse experience, with at least 1 year in Home Health.
- Proficiency in Oasis and Homecare Homebase documentation systems.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and interpersonal abilities.
- Reliable transportation and willingness to travel within designated service area.
- Active American Heart Association or American Red Cross BLS
