Sunbelt Staffing, a leading nationwide healthcare staffing agency, is partnering with a premier Home Health agency to find a compassionate and experienced Registered Nurse (RN) to join their team in Baton Rouge, LA.
Responsibilities:
- Conduct comprehensive patient assessments using Oasis and Homecare Homebase documentation systems to develop individualized care plans.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and IV therapy, ensuring compliance with physician orders and care plan directives.
- Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health outcomes.
- Collaborate with interdisciplinary healthcare team members to coordinate patient care and ensure continuity of care.
- Document patient encounters, treatments, and progress notes accurately and efficiently using electronic health record systems.
- Monitor patient progress closely, identify changes in health status, and adjust care plans accordingly to optimize outcomes.
- Adhere to all regulatory guidelines and agency policies to maintain high-quality standards of care delivery.
Job Details:
- Location: Baton Rouge, LA
- Schedule: Full-time, 40 hours per week, Monday to Friday, 8:00 am to 5:00 pm
- Occasional weekends & on-call.

Requirements:
- Valid Registered Nurse (RN) license in the state of Louisiana.
- Minimum of 2 years of Registered Nurse experience, with at least 1 year in Home Health.
- Proficiency in Oasis and Homecare Homebase documentation systems.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and interpersonal abilities.
- Reliable transportation and willingness to travel within designated service area.
- Active American Heart Association or American Red Cross BLS
