Job Description:
We are currently seeking a highly motivated and experienced Registered Nurse with a specialty in Home Health to join our team. The ideal candidate will have a state license, previous adult Home Health experience, and be knowledgeable in using Home Care Home Base and Oasis.
Responsibilities:
- Conduct patient assessments, develop and implement care plans, and provide skilled nursing care to patients in their homes.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure positive outcomes.
- Educate patients and their families about their healthcare needs and provide guidance on self-care techniques. •Communicate with the healthcare team to ensure continuity of care and to address any changes in patient status.
- Meet productivity goals and ensure all documentation is accurate and completed in a timely manner.
Requirements:
- Active state nursing license
- Minimum of 2 years of adult Home Health experience
- Proficiency in using Home Care Home Base and Oasis
- Required: Home Care Home Base, Oasis
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation and valid driver's license
- COVID-19 vaccination (medical/religious exemptions only)
- Must meet 30 productivity points
- OT preapproved by branch
- No on call unless stated upfront
