Are you a dedicated and experienced Registered Nurse seeking a rewarding position in Home Health care? We have an exciting opportunity for a Home Health RN in Hazlehurst. Join our dynamic team and provide high-quality care to patients in the comfort of their own homes.
Responsibilities:
- Deliver comprehensive nursing care to patients in a Home Health setting
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor and document patient progress
- Collaborate with interdisciplinary teams to ensure coordinated care
- Educate patients and families on self-care techniques and health management
- Maintain accurate and up-to-date patient records
Qualifications:
- Valid and current Registered Nurse (RN) license
- Minimum of 6 monthsof Home Health experience
- Proficiency in using Oasis documentation
- Experience with HCHB electronic medical record system preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Valid driver's license and reliable transportation
