Responsibilities:
- Drive to prospective patients’ residences.
- Provide the initial all-system nursing assessment prior to the provision care.
- Complete initial, on-going comprehensive assessments and chart observations of the patient’s condition and of the patient’s needs at each visit.
- Initiates the plan of care and necessary revisions including obtaining written physician orders for changes.
- Administer case management & follow-up skilled nursing visits in the patient’s home.
- Provide service and/or treatments requiring substantial and specialized nursing skills.
- Initiates preventive and rehabilitative nursing procedures (Home Health).
- Coordinate appropriate care, encompassing various healthcare personnel (such as Physicians, Physical Therapists, Occupational Therapists, Home Health Aides, and external providers) that are in the patient’s care plan.
- Oversee/supervise Home Health Aides, LPNs, and other Personal Care Workers according to law, regulation, and agency policy.
- Effectively plans and organizes work assignments, manages and sets competitive priorities, and completes work assignments with a minimum of direct supervision and within established time frames.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Self-development: Selects and attends education programs to keep current in area of practice. Utilizes new knowledge and skills. Participates in in-service programs. Annually prepares realistic professional goals and reviews with Supervisor.
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
Qualifications:
- Hold a current Registered Nurse (RN) license in the State of California.
- Possess and maintain a current CPR certification.
- Basic computer skills.
- Home health care experience preferred.
- Experience in wound care, vacs care, ostomy care, and IV administration is preferred.
- Demonstrates ability to work independently with minimal supervision.
- Fluent in Bilingual English and Cantonese/Mandarin preferred.
Job Requirements:
- Must have a valid driver’s license, and auto insurance.
- Required to present a negative TB test and other tests as required by agency policy
- Must be able to do field work.
- Must have excellent written and verbal skills.
- Excellent planning, organization, and presentation skills.
- Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
- Must be in good health and be able to fulfill the physical requirements of the job.
- Must be able to lift a minimum of 25lbs.
- Full COVID-19 vaccination required.
