Job Description: We are currently seeking a compassionate and skilled Registered Nurse to join our team as a Home Health RN in Clearwater. The ideal candidate will have a passion for providing high-quality care to patients in a home setting. As a Home Health RN, you will be responsible for assessing patients' health needs, developing individualized care plans, and delivering comprehensive nursing care to patients in their homes.
Responsibilities:
- Conduct initial assessments of patients' health status and develop personalized care plans
- Provide skilled nursing care, including wound care, medication administration, and symptom management
- Educate patients and their families on proper self-care techniques and disease management strategies
- Collaborate with interdisciplinary team members to ensure continuity of care and optimal patient outcomes
- Monitor patients' progress and make necessary adjustments to care plans as needed
- Document all patient care activities accurately and in a timely manner
- Communicate effectively with patients, families, and other healthcare providers
Qualifications:
- Valid Registered Nurse (RN) license
- Minimum of 6 months of experience in Home Health
- Oasis experience required
- HomeCare HomeBase experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Reliable transportation and a valid driver's license
- BLS Certification
