- Collaborate with behavioral health professionals to address mental health needs of patients
- Coordinate discharge planning to ensure smooth transitions from hospital to home or other healthcare settings
Requirements:
- Valid Registered Nurse (RN) license in [state]
- Proven experience in case management or a related field
- Strong knowledge of physiology, medical conditions, and treatment modalities
- Ability to conduct comprehensive patient assessments and develop individualized care plans
