Responsibilities:
- Provide case management services to patients, ensuring the coordination and delivery of high-quality healthcare services
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Collaborate with healthcare professionals to coordinate diagnostic evaluations and treatment plans
- Monitor and evaluate patient progress, adjusting care plans as necessary
- Utilize knowledge of physiology and medical conditions to provide comprehensive patient care
- Implement utilization management strategies to optimize healthcare resources and promote cost-effective care
- Provide patient education and support to enhance their understanding of their healthcare condition and treatment options
- Collaborate with behavioral health professionals to address mental health needs of patients
- Coordinate discharge planning to ensure smooth transitions from hospital to home or other healthcare settings
Requirements:
- Valid Registered Nurse (RN) license in [state]
