Job Description
**Job Summary:**
As a Home Health RN, you will be responsible for delivering high-quality nursing care to patients in their homes. This role requires a compassionate, dedicated nurse with a strong commitment to patient-centered care and the ability to work independently in a home setting.
**Key Responsibilities:**
- **Patient Care:** Assess patients' health problems and needs, develop and implement nursing care plans, and maintain medical records.
- **Medication Management:** Administer medications, track prescriptions, and educate patients and families on proper medication and treatment regimens.
- **Wound Care:** Provide wound care and instruction on wound management.
- **Monitoring Vital Signs:** Monitor and record patients' vital signs and report any significant changes to the physician.
- **Patient Education:** Educate patients and their families on disease processes, treatment plans, and health maintenance.
- **Coordination of Care:** Coordinate with physicians, therapists, and other healthcare professionals to ensure comprehensive care.
- **Home Safety Evaluations:** Assess the home environment for safety risks and recommend necessary changes.
- **Emergency Response:** Recognize emergency situations and respond appropriately.
- **Documentation:** Maintain accurate and detailed reports and records.
**Qualifications:**
- Current RN license in the state of practice.

- CPR certification.
