- Completing nursing process in delivery of patient care which includes assessment, planning, implementation, and evaluation of patient outcomes.
- Total oversight of patient from admission to discharge.
- Developing plan of care based on findings.
- Serving as patient’s advocate and maintaining ongoing communication with physician, referral sources, caregivers, and facility management to facilitate coordination of care.
- Timely completion of all skilled nursing documentation.
- Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
- Multidisciplinary team approach (coordinating with all disciplines of patient care, including PT, OT, ST, MSW, HHA's, and office staff)
Requirements:
- Current Illinois State RN.
- CPR Certification
- Previous experience in nursing or other health field experience
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem-solving and critical-thinking skills
- Ability to thrive in a fast-paced environment
- WellSky EMR knowledge preferred, but not required.
Job Types: Full-time, Part-time
Pay: $70.00 - $85.00 per hour
Expected hours: 15 – 40 per week
