• Overtime Multiplier: 1.25% for 40 total hours in 1 week
• Charge Nurse: $3.00
• On Call: $5.00
• Call Back: 1%
• Holiday: 1.25%

Job Responsibilities:

As a Physical Therapist in the Home Health setting, your responsibilities will include:

• Conducting patient assessments and developing individualized care plans.
• Providing physical therapy services to patients in their homes.
• Monitoring and documenting patient progress.
• Collaborating with healthcare professionals and family members to ensure effective patient care.
• Adhering to all relevant healthcare regulations and standards.
• Providing education and support to patients and their families.
• Maintaining accurate patient records.

Description:

We have an exciting opportunity for a dedicated Physical Therapist to join our Home Health team  in Greenville, KY.

Start Date: ASAP

Length: 13 weeks renewable

Requirements:

• Licensure and certifications – PT, BLS
• Education – PT
• Experience – 1-2 years preferred, but new grads are welcome
