The Physical Therapist evaluates and treats patients in accordance with orders from the physician through the use of therapeutic procedures designed to address deficits in physical function and improve independence, safety and quality of life. Ensures quality and efficient therapy is provided to patients by managing the delivery of care throughout the course of treatment and delegating appropriate components of care to the Physical Therapist Assistants under his/her supervision. Direct and aid patients in increasing gait, balance and functional independence through active and passive therapeutic exercise, muscle reeducation, gait and functional training and transfer activities. Participates as a member of the interdisciplinary team to provide support and information within the area of physical therapy.
Responsibilities and Duties:
- Evaluate and develop individualized patient treatment plans.
- Collaborate with clinical staff, patients, and families to maximize functional abilities.
- Document treatment progress clearly and concisely, including discharge summaries.
- Provide instruction to Physical Therapist Assistants and offer supervision.
- Collaborate with the clinical team to develop and educate patients and caregivers on safe discharge plans.
Requirments:
- Graduate of an accredited Physical Therapy curriculum.
- Current or able to get license/certification in Wisconsin.
- Current or able to get American Heart Association CPR Certification
- Minimum 1 year experience in Home Health Physical therapy
