Trusted Ally Home Care is a growing multi-state home care agency that specializes in providing nursing and Home Health aide services to nuclear-exposed employees in the comfort of their homes. TAHC has been serving families and their loved ones since 2010. After witnessing the impact that quality home care services brought to her grandfather's life, our co-founder Candace Honeywell, was determined to bring the same level of care to everyone we serve.
Role Overview: A Registered Nurse (RN) Team Lead administers & oversees skilled nursing care to patients on an intermittent basis in their place of residence. Supervises teammates to ensure the Plan of Care is being followed in accordance with physician orders and under the direction and supervision of the Senior Case Manager/Nursing Supervisor.
Job Duties:
Creates, initiates and updates the Plan of Care in accordance with State Regulations.
Provides services in accordance with the Plan of Care.
Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs. This includes the completion of certification/recertification orders and discharge summaries as needed.
Initiates appropriate preventive and rehabilitative nursing procedures.
Prepares clinical and progress notes for each patient visit and summaries of care conferences on patients in a timely manner as per Agency policy.
Coordinates additional services needed.
Informs physician and other personnel of changes in the patient’s condition and needs.
Oversee Durable Medical Equipment Orders.
Work with clinical scheduler to ensure schedule is complete and address any gaps in coverage in addition to any “Meet and Greets” that need to be scheduled.
Approves the PTO or sick time requested through ADP that follows company policy each week for all direct reports.
Counsels the patient and is primary point of contact for any concerns by family and/or MPOA.
Participates in the in-service programs, staff meetings and patient care conferences.
Processes orders and notifies physician of patient needs and changes in condition.
Completes incident reports and/or medication errors and reports to the Treating Physician and Nursing Supervisor.
Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker of patients requiring their specialized skills.
Supervises the LPN/LVN every 60-days and supervises the HHA/CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel. This is documented in IGEA as a standalone form addressing any questions or concerns around the plan of care.
Conducts patient care conferences on patients assigned to his/her care.
Participates in peer reviews, annual reviews, annual skills competency checklists and Quality Assessment Performance Improvements assigned.
Participates in utilization review of medical records as assigned.
Takes on-call duty nights, weekends and holidays, as assigned.
Completes and submits head-to-toe assessments, reassessments, and transfers, resumptions of care, discharges and significant changes in condition in accordance with Agency defined time frames (48 hours).
Appropriately utilizes ICD-10 diagnoses and codes.
Observes confidentiality, safeguards all patient related information.
Attend staff meetings and patient care conferences as scheduled.
Immediately report any patient incidents/variances or complaints to Senior Case Manager/Assistant Director of Nursing
Assists the team with clinical questions and oversight.
Performs other duties as assigned.
Education and Experience Qualifications:
Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
One (1) year nursing experience preferred.
Acceptance of philosophy and goals of this Agency.
Ability to exercise initiative and independent judgment.
Maintain current CPR certification, licensure and licensure requirements.
