Home Health Nurse: RN / LVN

We are seeking a Home Health Nurse- RN & LVN to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

• • Responsible for visiting patients for physician ordered in home care.

  • Plan, organize and direct home care services

  • Complete initial assessment of patients to determine home care needs

  • Regularly re-evaluate patient’s nursing needs

  • Initiate a plan of care and make revisions as patient status changes

  • Administer medications and treatments as prescribed by physicians

  • Monitor and record patient vital signs and any changes in medical status

  • Provide skilled care including wound care, dressing changes, wound vac, etc

  • Conduct patient education and provide support to patients and their families

  • Collaborate with other healthcare professionals to implement patient care plans

  • Maintain accurate and up-to-date medical records using electronic health record.

  • Follow aseptic techniques to prevent infection transmission
  • Assist with discharge planning at end of care

• Minimum Qualifications

  Licenses/Certifications:

• Current license as a Registered Nurse or Licensed Vocational or Practical Nurse in the State of California required
• Current CPR certification required

• Experience:

• Minimum of one (1) year of nursing or healthcare experience required
• Home health care experience is strongly preferred

• Skills and Abilities:

• Excellent communication & interpersonal skills to effectively interact with patients, families, and healthcare team members- written and verbal, organizational, presentation and facilitation skills
• Highly computer literate; tech savvy a must
• Familiarity with medical software and equipment
• Ability to speak, read, and write English
• Bilingual- a plus
• Self-directed with the ability to work with little supervision, prioritize tasks, manage time efficiently, and work well under pressure
• Flexible and reliable in fulfilling role obligation
• Maintains confidentiality in all aspects of the job
• Schedule flexibility is necessary to meet the needs of patients
• Previous experience in nursing or other medical fields
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Strong understanding of aseptic technique and infection control practices

Pay: LVN: $40.00 - $45.00 per hour / RN: $120+
