Registered Nurse (RN) or LVN - Home Health Nurse
ABL Health Care, LLC the Home Healthcare agency is seeking an experienced Registered Nurses (RN) or Licensed Vocational Nurse (LVN) with at least 1 year of Home Health Quality metrics experience. MUST be an expert in using quality tool SHP (STRATEGIC HEALTHCARE PROGRAMS) for tracking quality outcomes and working with Leadership to implement Quality programs based on SHP data.

Responsibilities:
- On continues basis utilyze SHP and Kinnser tools to access staff/company performance and develop action plans for continues growth and star rating improvements.
- The Quality Assurance Nurse SHP, a certified and professional nurse, holds the responsibility of scrutinizing the consistency and accuracy of Home Health and Hospice documentation and evaluation processes using SHP and Kinnser tools.
- This role involves proactively examining clinicians' record-keeping to ensure it aligns with federal and state regulations and conforms to organizational policies regarding appropriateness and thoroughness. In case of any discrepancies, they collaborate with clinical staff to rectify data integrity issues as per organizational guidelines.
- Moreover, the Quality Assurance Nurse evaluates visit schedules to ensure they adhere to care guidelines and patient needs. They also highlight potential financial risks or instances of underutilization to the clinical manager or designate for further action.
- A core aspect of this position involves advocating for and advancing the principles of continuous Quality Assessment Performance Improvement (QAPI) and supporting the integration of new staff into the Home Health and Hospice QAPI program including management of outcomes using SHP tools.
- This professional actively engages in various Quality Improvement and Corporate Compliance initiatives as assigned and contributes to the development and implementation of policies that meet regulatory standards and laws.
- They play a pivotal role in conducting studies and administrative tasks for the QAPI program and serve as a model of excellence for colleagues by exemplifying high standards in behavior, appearance, cooperation, and job performance.
- Additionally, the Quality Assurance Nurse participates in activities preparing for surveys and audits, such as mock audits and training sessions, and contributes to the formulation and implementation of corrective action plans.
- They provide crucial advice to leadership on clinical compliance concerns and continually evaluate and interpret quality and accreditation standards to ensure adherence through reporting and measurement methodologies.
- In cases where on-site visits during accreditation surveys are necessary, this role collaborates in the development of correction plans and monitors their implementation.
- Furthermore, the Quality Assurance Nurse collaborates with the QAPI Director and the DPCS of Clinical Services for each program to assess specific needs and ensures adherence to established Hospice and Home Health policies and procedures.
- Continual professional development is vital in this role, achieved through attending educational workshops, reviewing professional literature, and participating in additional chart audit tasks as assigned.
Qualifications:
- 1 year of hands on experience with SHP (STRATEGIC HEALTHCARE PROGRAMS) Tools
- 1 year of hands on experience with Kinnser
- Licensed nurse with a valid state nursing license
- Mandatory 1-2 years of hands-on experience in clinical Home Health
- Familiarity with federal regulations and state licensing criteria
- Proficiency in using computer systems
- Possession of a functional and insured automobile, along with a valid driver's license as per organizational standards
- Strong coordination and communication abilities
- Attentive to details and capable of working with minimal supervision
- Required to clear criminal background check and drug screening
- Location in Burlingame, CA 94010: Must reliably commute or intend to relocate before starting the job (Mandatory)
- Minimum one-year experience in quality assurance (Mandatory)
