- PURPOSE OF JOB:
The Intake specialist performs a variety of duties. Primary responsibility is for intake of all referrals from sources outside and inside Doylestown Hospital. This position requires skill in planning, organizing, and coordinating the initiation of home care services.
II. ESSENTIAL FUNCTIONS:
- Accepts appropriate referrals to VN/ Home Health after screening for appropriateness and safety with input from the Intake Nurse/Clinical Manager as needed for clinically complex cases. Processes according to established standards, policy, and regulations. Keeps up to date with all necessary requirements and changes in order to complete work correctly. (25%)
- Uses computer to assure current and necessary information is available for clinicians and other office personnel. (25%)
- Provides accurate and pertinent clinical information on referral of patients. This includes primary diagnosis, ICD 10 code for the diagnosis, and any procedure ordered on referral such as wound care or Coagu check INR. (25%)
- Demonstrates fiscal responsibility by assisting in handling initial Face To Face documentation and contacting Primary physician for the Home Health episode, and pursuing and receiving authorizations for services from payors when asked. (25%)
Job Qualifications:
QUALIFICATIONS:
- Education: High School or equivalent is required, some college or technical training preferred.
- Experience: 1-2 years office/hospital experience or data entry experience. Basic computer skills required. Knowledge of medical terminology and Home Health coding a plus.
C. Other Skills: Able to work as part of a team. Ability to manage effectively to meet program goals and objectives. Proficiency with computers, data entry. Demonstrates attention to detail. Demonstrated inter-personal and communication skills. Excellent customer service skills. Strong organizational skills. Ability to work independently and at a fast pace. Ability to multi-task and prioritize tasks. Negotiation and problem solving skills. Good understanding of reimbursement by Medicare, Medical Assistance and Commercial Insurance. Good understanding of Federal, State and JCAHO regulations. Must maintain current Pa. driver's license and auto insurance to maintain employment in the VN/HC department.
