General Summary: To ensure seamless patient transition into agency's services using clinical skills by interfacing with hospital/facility staff, physicians, and families to coordinate and process patient referrals.
Essential Functions:
- Acts as a clinical specialist that represents the agency via telephone with referrals for patient care from physicians, hospitals, insurance case managers and other sources in the community.
- Always responds to communication regarding potential referrals for services with a professional service orientation.
- Applies professional expertise to accurately completing referral documentation as necessary to facilitate the admissions process.
- Reviews documentation, including orders, face to face encounter visit, and physician who will follow the patient in the community. Contacts referral source for clarification of orders or other documentation to determine clinical eligibility for care and confirm/verify required information.
- Serves as a liaison between the referral source and agency regarding referral of patients who are in need of direct admission or who are in need of home care services upon discharge from the hospital.
- Ensures contact with patient/family as soon as possible after referral is received.
- Works closely with agency leadership, Business Development team, and peers to support the day-to-day operations of the team.
- Works closely with Clinical leadership to facilitate discussion about referrals during the referral-to-admission process.
- Processes and documents referrals that are not accepted for admission, indicating specific reason for non-acceptance.
- Utilizes current and/or department specific software to complete assignments.

Experience:
- Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations.
- Must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the director.
- Must have knowledge of current home care admission criteria, Medicare, Medicaid, State and Federal regulations.
- Must be a licensed professional by the State Board of Nursing.
- Have had two (2) years of experience with a HOME HEALTH, medical, surgical, or critical care area.
ADDITIONAL DESIRABLE QUALIFICATIONS
- Must be proficient in the use of Microsoft programs for internal communications (email) and database tracking/reporting, as well as proficiency working with electronic health records.
- Must have excellent oral and written communication skills and a strong commitment to customer service.
Skills:
- Evidence of progressive leadership responsibilities, including fiscal management is required.
- Demonstrated knowledge of HOME HEALTH licensure, regulations, and Medicare Conditions of Participation (COP).
- Excellent communication skills.
- Excellent interpersonal skills necessary in order to communicate effectively with a wide variety of personnel when identifying and resolving sensitive and complex patient or administrative issues.
- Nursing skills as defined as generally accepted standards of practice.
- Excellent interpersonal skills and ability to communicate effectively.
- Demonstrates proven decision-making skills.
- Must read, write and comprehend English.
Licensure/Certification:
- License and current registration as a Licensed Professional or Vocational Nurse (LPN/LVN).

Pay: $26.00 - $28.00 per hour
