NIGHT, DAY, EVENING, WEEKEND SHIFTS are available.

Individuals must have NYS HHA certificate in good standing with NYSDOH.

Potential employee must be vaccinated for Covid-19 in accordance with Section 16 Orders by NY Governor mandating Covid-19 vaccinations for healthcare workers including hospitals and long-term care facilities staff members.

Must Have:

1. Valid Original HHA Certificate *NY STATE*.

2. Valid Identification and Proof of Authorization to Work.

3. Two references.

4. Physical Examination (Must be done within the recent three months), PPD/QuantiFERON TB (Ideally be done within the recent three months), Drug Test (Must be done within the recent three months), Measles, Mumps, Rubella Lab Report.

5. Covid Vaccination Card.

Performance requirements include but are not limited to:

1. Provide care for assigned residents, which will include health care tasks, personal hygiene services, housekeeping tasks, and other related support services essential to the resident's health and wellbeing.

2. Follow care plan and instructions to meet resident's needs.

3. Observe, record and report any and all changes in resident condition to shift supervisor in timely fashion.

4. Participate in staff meetings and In-service programs.

5. Maintain a friendly, positive, enthusiastic attitude to facilitate a homelike and cheerful atmosphere.
