Summary: We are seeking a Home Health Aide to provide routine, personal healthcare service to developmentally disabled persons temporarily assigned to hospitals, skilled nursing facilities, rehab centers, etc. within Bronx and Manhattan as needed

Description of Roles and Responsibilities:
· Provide companionship, engage in meaningful activities, escorting the individual to hospital procedures and advocate for the individual
· Assist in Activity of Daily Living functions (eating, bathing, dressing, toileting, transferring (walking) and continence) in accordance with the medical treatment plan provided and the reason for hospitalization
· Assist with feeding as per prescribed diet order and monitoring diet consistencies
· Assist in routine health care functions under the supervision of a nurse at the assigned hospital. (These Aides will not write on the hospital chart; however, they will document on an appropriate Logs)
· Aide must follow the individual’s plan of care to ensure safety
· Aide must remain alert and awake during requested tour
· Aide must inform the Nurse Administrator/Designee at the site of service whenever the aide needs to take a break from service during the shift whereby the Nurse Administrator/Designee will assign hospital coverage
· Aide must inform both the contact and the administrator at the site of service of any reportable incidents that occur during the time of service. (Contractor will be provided guidance on the types of incidents that must be reported to the hospital administration by the treatment team)
Mandatory/Minimum Qualification, Skills, Experience Requirements:
High School and/or Technical Training as a minimum educational requirement

· Reliable transportation
· Must be at least 18 years old
· Must be able to pass a criminal background check
· Must be able to provide references
· Ability to work with limited supervision
· Ability to follow written and verbal instructions
· Have good personal hygiene
· Able to do light housekeeping
