WHAT YOU CAN EXPECT

• Assist patients with care and activities of daily living as directed in the established Plan of Care
• Complete necessary documentation of care provided.
• Records intake, output, and vital signs as directed and communicates care as appropriate.
• Provide assistance with personal hygiene and elimination needs according to the established plan of care.
• Assist patients with ambulation and transfer using proper body mechanics.
• Communicate any change in condition or lack of patient progress.
• Schedule visits with the patient or caregiver.

QUALIFICTIONS

• High School Diploma/GED - Required
• 1 year Home Healthcare - Preferred
• 1 year Long Term Care Setting - Preferred
• Certified Nursing Assistant (CNA) - Preferred
• Home Health Aide (HHA) - Required
• Cardiopulmonary Resuscitation (CPR) - Upon Hire Required
• Auto Insurance - Upon Hire Required
• Driver's License - Upon Hire Required
