Home Health Aide - Home Health
ABL Health Care, LLC the Home Health agency is seeking an experienced Home Health Aide (HHA) with at least 1 year of Home Health experience. By joining our team providing the highest quality experience for patients in the comfort of their own home.
Position Overview: The Per Diem Home Health Aide (HHA) will operate within a Home Health setting and will be assigned responsibilities that prioritize the quality, continuity, and safety of patient care. The HHA's primary responsibility is to observe and deliver patient care services to hospice patients.
Responsibilities:
Provide personal care to Home Health patients based on the care plan or supervisor's instructions, including tasks like feeding, dressing, bathing, toileting, personal hygiene, changing bed linens, and laundry.
Document observations related to the patient's condition, care provided, and any significant changes.
Ensure the patient's safety and operate durable medical equipment (DME) and other devices safely and correctly (e.g., hospital beds, side rails, oxygen equipment, nebulizers, footstools).
Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as needed.
Carefully monitor and maintain the patient's skin health to prevent skin breakdown.
Assist with ambulation, range of motion exercises, and other activities as directed by the Supervisor.
Offer respite care for patients, family members, or caregivers when on-site, as appropriate.
Maintain cleanliness and orderliness in the patient's living area as designated.
Show respect to patients, family members, and their personal needs.
Record HHA visits in the Electronic Medical Record (EMR) system and follow the HHA care plan.
Attend monthly staff meetings and in-service training sessions.
Keep required competencies and licensure up to date to perform the job effectively.
Perform other duties as assigned.
Qualifications:
Possession of a current California Home Health Aide (HHA) license.
Current CPR certification.
One year of prior experience as an HHA.
A team player with strong organizational, communication, and problem-solving skills.
Ability to read and accurately document care provided.
Capability to follow both written and verbal instructions.
Demonstrates tact and patience in patient care.
Understanding of the hospice philosophy and comfort in providing specialized care to terminally ill patients.
Physical Abilities/Requirements:
Ability to safely lift or transfer up to 50 lbs, with or without assisted devices (including patients and supplies).
Capability to push, pivot, pull, and reposition up to 50 lbs, with or without assisted devices.
Flexibility to bend, squat, stoop, crouch, kneel, reach, climb, and rotate as needed.
Manual dexterity (grasping, gripping, pinching).
Capacity to stand and walk for extended periods.
Ability to drive for extended periods when necessary.
Adequate visual and hearing acuity, with or without assisted devices.
Adaptability to various work environments, including internal office settings with noise and interruptions, open workspaces, and external settings such as different community and living environments, varying weather conditions, and travel/traffic conditions.
